Ageless Med Spa Membership Program

Terms and Conditions

These Terms and Conditions (“Terms”) govern your participation in the Ageless Med Spa Membership Program (“Membership”), administered by Ageless Med Spa (“we,” “us,” or “our”). By enrolling, you (“Member” or “you”) acknowledge and accept the policies outlined herein. These Terms apply for the full duration of your membership and may be amended from time to time.

1. Membership Overview

• Ageless Med Spa offers tiered membership options designed to support personalized aesthetic, wellness, and skincare goals.
• Each membership tier includes a curated package of benefits such as monthly treatments, product discounts, exclusive pricing, loyalty rewards, and early access to special promotions.
• A membership is valid for a period of 12 months beginning from the date of enrollment and is renewed monthly unless canceled as described herein.

2. Eligibility Requirements

• Must be at least 18 years of age and a legal resident of the United States.
• Membership is intended for individual use only. Shared or transferred use is not permitted.
• We reserve the right to reject applications or revoke memberships at our discretion for violations of spa policies or abuse of the program.

3. Enrollment & Payment Terms

• Enrollment may be completed online via the Ageless Med Spa app or in person at our Anchorage location.
• Upon enrollment, Members must provide a valid credit or debit card. This card will be securely stored for monthly auto-pay billing.
• Membership dues are charged on a recurring monthly basis for the full duration of the 12-month agreement.
• Select tiers may require a one-time, non-refundable activation fee at sign-up.
• Members are responsible for ensuring that their payment information remains up to date. Failed or declined payments may result in suspended access to benefits until the balance is settled.

4. Cancellation Policy

• Memberships are a 12-month contractual commitment, though Members may cancel at any time.
• Early cancellations (prior to the end of the 12-month term) will incur a $399 early termination fee.
• To cancel, a written request must be submitted via email to info@agelessms.com or completed in person. The request must include the Member’s name, contact information, and reason for cancellation.
• Cancellation is considered complete only upon written confirmation by Ageless Med Spa.
• Members who cancel will forfeit any unused treatments, credits, or membership-exclusive discounts.

5. Membership Freeze (Temporary Suspension)

• Members may request a freeze of their membership for up to 2 months during the 12-month term.
• Freeze requests must be submitted in writing and approved by management.
• During the freeze, all benefits are suspended, and no monthly dues will be charged. The membership term will be extended by the duration of the freeze.
• Freezes are not permitted retroactively and are not available to Members with outstanding balances.

6. Access to Benefits & Usage Guidelines

• Members must remain current on payments to access any membership perks.
• Complimentary or monthly-included treatments must be redeemed within the corresponding billing cycle. Missed services do not roll over unless otherwise stated in writing.
• Member-only pricing cannot be combined with promotional offers unless explicitly authorized.
• Benefits are valid only while the membership is active and cannot be reinstated after cancellation.

7. Refund Policy

• All membership fees, including activation fees and monthly dues, are non-refundable under any circumstances.
• There are no credits or reimbursements for unused services, missed appointments, or canceled sessions.
• Prepaid services or accrued discounts lose their value upon cancellation.

8. Program Modifications

• Ageless Med Spa reserves the right to modify or update any part of the membership program, including but not limited to: pricing, benefits, treatment options, or terms of use.
• In the event of a material change, Members will be notified via email, SMS, or in-app notification at least 30 days in advance.
• Continued use of the membership after such notice constitutes acceptance of the revised terms.

9. Termination by Ageless Med Spa

• We reserve the right to terminate a membership at any time for any of the following reasons:
  • Failure to pay dues
  • Abuse of staff or violation of spa policies
  • Misuse or fraudulent use of benefits
  • Conduct deemed harmful to the business or clientele
• Upon termination, any remaining benefits are forfeited without refund or credit.

10. Legal & Dispute Resolution

• These Terms shall be governed by the laws of the State of Alaska.
• Any disputes arising under or related to this Membership must be resolved exclusively through binding arbitration. Members waive the right to sue in court or join class action lawsuits.
• Small claims matters may be resolved in a court of competent jurisdiction in Anchorage, Alaska, if eligible.

11. Acknowledgment & Acceptance

By enrolling in the Membership Program, you acknowledge that you have read, understood, and agree to abide by these Terms. You further agree to receive Membership-related communications and accept responsibility for managing your account and appointments.
